Answers:
I'm assuming you mean a Foley catheter and it should be below the waist line, most people are supplied with a "leg bag" where there is an attachment to apply to your leg that you can hang the bag from. It should be draining the bladder quite continuously and if it is not draining urine, it needs to be checked by a nurse or doctor for a possible kink or clog.

It should be below the catheter. It should come out instantly. If not, there may be a blockage. Just keep the bag lower than the bladder. If you're having a problem, call a visiting nurse.
I assume you are talking about a person who has an indwelling urinary catheter - ie. a tube inside their bladder to drain urine - with a bag attached. It doesn't really matter where you put the urine bag - but I suppose ideally you should hang it from the bed frame, or if you can't do that - allow it to lay on the ground. If the catheter is placed correctly, as soon as it enters the bladder, urine should come out. Actually, the catheter should drain the entire bladder almost immediately. Why do you ask?
