his company that he works for he is opted out of their coverage and they are requesting verification

Speak with whoever is in charge of your employee insurance coverage. Verify that your husband has been added to your coverage. Make sure that person has the authority to send such verification to your husband'semployer. Ask for a copy of that letter. Ask that it be done ASAP and follow up about a week later to see if it has been done. Keep after the responsible person until the letter is sent. Make waves if you must but otherwise, be nice.. Also check with Payroll that proper deductions are made if necessary.

You need to contact your insurance provider and they should send you a statement verifying his coverage under your plan. That is your first step. They may even fax it to your husband's boss so that he can get a first hand copy.
You need to call your insurance company, on the back of your insurance card and ask them to fax you verification of your husband being on there and the effective date. or have them phone the info to his company
