That depends upon where you work (if you are looking at insurance through your workplace). If not, you are going to have to do some major investigating.
Your best bet is with an employers group plan, preferable a state, federal, or city plan. If that is not possible try to find one in which the premiuim doesn not go up,. Get one with a larger deduction as you are going to pay with either the deduction or the premium. Unfortunately the days of cheap policies are gone.
Pick a company which has been around a lng time , like nation wide, GE. farm bureau, state farm ect.
Do you work? If so, your Human Relations office should have provided you with some info on various insurance companies. If not, you can call different companies like, Blue Cross-Blue Shield, Kaiser Ppermanente, Etna, etc.. and inquire their premiums. A lot of companies also carries dental plans.
