Daily.
Once or twice a day.
Daily before I go to bed.Don't want to give all that nasty bacteria a chance to play around for 8 hours while I sleep
Twice daily and for optimal hygeine after every meal following brushing.
once a day before sleep
Hardly ever. I know, I'm so bad!

As much as needed to get them clean. I believe that flossing is more important then brushing. Your lips and tongue provide some cleaning support to the tooth surface that is exposed but that is only about 60% the other 40% is in between where your teeth are touching, that is also the place where most adults get cavities.
