Nowhere. That is determined by your insurance company and it is usually based on where your dentist's office is located. For example, if your dentist is located in the downtown area, it is expected that your dentist should be compensated a higher amount. If you have the option of choosing a PPO plan, you can usually obtain a fee schedule which tells you the maximum allowable fee for each procedure and it includes the procedure code. However, if you have a question about UCR fees for things that you may have done repeatedly, like an exam, bitewings (x-rays of your back teeth) prophylaxis (routine cleaning), you may want to refer to the EOBs (Explanation of Benefits) that your dental insurance carriers sends you. Remember everything that your insurance sends your dentist, they send you a copy as well.
